Requirements

 

Registration of Birth

 

1

Report the occurrence of birth to the barangay secretary where the child was born for the preparation of birth certificate.

Submit the birth certificate to the municipal civil registrar.

 

If delayed: Brgy. Certification Baptismal certificate, immunization card & permanent school record.

Undergo interview

Submit supporting documents for delayed registration and wait for 10 days posting.

Pay the registration fee for legal instrument, certificate of registration and certification of birth at the  office of the municipal treasurer.

CENOMAR, Marriage Certificate and birth certificate and affidavit of legitimation.

Affidavit to use the Sure of the Father

Affidavit to use the surname of the father and birth certificate of the child

Pay for the registration fee for legal instrument, certificate of registration and certificate of birth bearing the annotation.

 

 

Out-of-town Registration of Birth

 

1

 

Undergo interview

NSO Negative certification of birth and at least 3 documents such as; Baptismal Certificate, Marriage Certificate, permanent school record, voters affidavit, employment record & other document showing the birth date and birth place.

Submit documents

Pay service fee at the office of the municipal treasurer.

Wait for the approval of the receiving municipal registrar

 

Registration of Death

 

 

 

 

1

Report the occurrence of death to the barangay secretary where the person died for the preparation of death certificate.

 

Brgy. Certification & Certification of priest if delayed

Forward the death certificate to the Mun. Health officer within 48 hours.

Submit the death certificate to the Municipal Civil Registrar.

 

IF DELAYED REGISTRATION

Submit supporting documents for delayed registration and wait for ten days posting.

Application for Marriage License

Undergo interview

 

CENOMAR, Birth Certificate parents’ consent, and Pre- marriage Counseling

Comply requirements

Pay Application fee for marriage license and marriage license fee at Municipal Treasury office

Wait for 10 days posting

Registration of Marriage

1

Submit certificate of marriage duly signed by contracting parties, solemnizing officer and witnesses.

Issuance of Certification

Petition for correction of clerical error

Document to be corrected; two documents bearing correct entries such as: Birth certificate and marriage certificate; voter’s affidavit; employment record; GSIS record; SSS RECORD; Medical record; Business record; School record; Driver’s license; insurance; Civil registry records of ascendants; land titles; Certificate of land transfer; Bank book; NBI/Police clearance and others.

Pay the petition fee at the municipal treasury office

Wait for ten days posting

After posting, wait for the affirmation from the office of the Civil Registrar General.

 

After affirmation by CRG

Pay for the certification of birth with annotation/ remarks and for the Certificate Finality

 

Petition for Change of First Name

1

Undergo interview

Birth certificate and at least two documents bearing the use name; Baptismal certificate; Voters affidavit; Employment records; GSIS record; SSS record; Medical record; Business record School record; Driver’s license

Submit requirements

Pay the petition fee at the office of the municipal treasury office

4

Publish in the news paper of original circulation at least once a week  for two (2) consecutive weeks

 

5

Wait for the affirmation from the office of the Civil Registrar General.

Insurance; Civil registry records of ascendants; Land titles; Certificate of land transfer; Bank book; NBI; Police clearance; Employer’s clearance; Affidavit of publication from the publisher; Copy of the news paper clipping and others.

 

After affirmation from CRG:

Pay for the Certificate of Finality and the certification with the annotations at the office of the municipal treasurer.
